*1 *There is no food safety manager certified by this Department (CFSM) on duty as required. A CFSM must be on duty at all times the facility is open and/or food is being prepared. A completed application(SA-71)for CFSM, or proof of enrollment in an approved safety class must be submitted to the Department.
Food license renewal will not be approved without certification or proof of enrollment in an approved food safety manager course.
 Repeat Violation.
*8 Employee observed washing coffee carafe in the front counter handwashing sink.
A handwashing facility may not be used for purposes other than handwashing.
Remove affected ware(s) to the 3-compartment sink.  New Violation. To be Corrected By: 07/21/2022
*11 *Half-gallon container of chocolate milk found offered for sale beyond 'sell-by' date (07-18-22) listed on the product.
Affected container was removed from the sales floor during inspection.
 Corrected On-Site.  New Violation.
*14 *1. Multiple dipensing spouts on the self-service beverage dispenser contain a lumpy, mold-like substance.
Detail clean and sanitize affected areas prior to use of unit.
2. Rear exterior of the ice chute, on the self-service beverage dispenser, is unclean with a grey, mold-like substance. Substance is dripping to bottom of ice chute.
All surfaces of the ice chute must be thoroughly cleaned and sanitized.

*19 *Sausage/Biscuit sandwich in the hold holding box (at the end of the front counter) was measured at 109°F.
Hot food must be held at 135°F or above.
Sandwich was discarded and unit temperature was adjusted during inspection.
 Corrected On-Site.  New Violation.
42 Tongs for use at the self-service Grab-and-Go hot holding units, are not tethered to prevent utensil from falling onto the floor.
Tether affected utensils to prevent contamination.  Repeat Violation.
53 A black, mold-like material is accumulating on the ceiling in the walk-in cooler.

General Remarks
Food license renewal application and fee have not yet been received by this Department. Current license will expire 08-09-22.

Food license renewal will not be issued unless facility employs staff who are (or who have registered for the course and exam) a certified food safety manager.
A follow-up inspection may be conducted to check on corrective actions taken in response to violations referenced in today's inspection report.
